Yoga Center of Columbia LLC (“Yoga Center”) owns the yoga studio located at 8590 Route 108, Suite 109, Columbia, MD 21045 (“Studio”). Yoga Center makes the studio available to Independent Contractor instructors to provide yoga instruction to their clientele. Yoga Center does not direct or supervise, and is not otherwise involved in the classes by the Independent Contractors who are each a distinct business enterprise carrying their own respective insurance coverages.
To the extent allowed by applicable law, I hereby release and agree to hold Yoga Center of Columbia LLC and the Independent Contractor yoga instructors harmless from any causes of action, claims, costs and expenses, including compensation for damages and harm that I sustain, caused by any act or failure to act, or that may otherwise arise in any way from any services received. I understand that this release discharges the aforementioned from any liability with respect to personal injury, physical and/or psychological, illness and/or transmission of communicable disease, death, medical treatment, or property damage that I may incur arising from, or in connection to, any services received at the Yoga Center. I acknowledge that I am voluntarily participating and assuming with the understanding of these risks and their outcomes. This waiver of liability and release extends to all owners, teachers, and employees.
I also affirm that I am healthy enough to participate in the activities that I choose to participate in that are offered at the Yoga Center.
